Terms of Service

1. Agreement and eligibility

By purchasing, accessing, or using the service, you agree to these Terms and the policies linked from checkout. You must be at least 18 years old, legally able to contract, and provide accurate information. If you do not agree, do not enroll.

2. The service

Re:forma is available as either (a) the Signature subscription, which includes an initial 120-day personalized coaching period and then continues monthly, or (b) the Flexible monthly coaching subscription. Both include an intake review, coach assignment, personalized nutrition guidance, a personalized training plan, weekly 1:1 coaching, reasonable plan adjustments, platform access and support, and guided educational materials while the service is active. Services are delivered remotely through video calls, messaging, and the Re:forma platform.

Unless checkout expressly states otherwise, supplements, food, gym membership, medical tests, devices, medical services, therapy, and emergency support are not included.

3. Start, scheduling, and participation

Your start date is confirmed during enrollment. After payment and completion of the intake, we normally provide onboarding, platform access, and the first plan within five business days. You are responsible for attending sessions, providing timely and truthful information, using safe equipment, and following any medical restrictions given by your clinician.

Sessions may be rescheduled with at least 24 hours’ notice, subject to availability. A missed session without timely notice may count as delivered. We will make reasonable efforts to reschedule a session cancelled by us.

4. Prices and payment

Prices are displayed in U.S. dollars before purchase. Signature costs $799 USD at purchase, which covers the first 120 days. Beginning on day 121, Signature renews at $199 USD per month until you cancel. Flexible costs $299 USD at purchase and renews monthly at $299 USD until you cancel. Taxes, if legally required, are shown before payment.

For either option, you authorize our payment processor to store your payment credential and make recurring charges only according to the schedule and consent shown at checkout. Your bank may apply currency conversion or international transaction fees that we do not control.

5. Cancellation and refunds

Our Refund & Cancellation Policy is incorporated into these Terms. Both options continue until cancelled and are governed by our Subscription & Recurring Payment Terms. Cancelling Signature before day 121 prevents the first $199 renewal; it does not by itself reverse the $799 initial payment. You may cancel through the Stripe Customer Portal or by emailing support@reforma.fit.

6. Wellness and safety

Read our Wellness Disclaimer. Re:forma does not diagnose, treat, cure, or prevent any disease. You are responsible for obtaining medical clearance where appropriate and stopping activity that causes pain, dizziness, shortness of breath, or other concerning symptoms. In an emergency, contact local emergency services.

7. Platform rules and intellectual property

We grant you a limited, personal, non-transferable right to use program materials during your participation. You may not copy, resell, publish, scrape, reverse engineer, share account access, harass staff or participants, or use the service unlawfully. Re:forma content, software, trademarks, and methods remain owned by us or our licensors. Your personal records remain subject to our Privacy Policy.

8. Suspension and termination

We may suspend or terminate access for nonpayment, material breach, abuse, unsafe conduct, or misuse of the platform. If we end the service without your breach, we will refund the undelivered portion. Sections that by nature should survive termination—including payment obligations for delivered services, intellectual property, disclaimers, and dispute terms—remain effective.

9. Disclaimers and liability

Results vary with health, circumstances, and participation. To the fullest extent permitted by law, the service is provided without guarantees of weight change, fitness gains, or other outcomes. We are not liable for indirect, incidental, special, consequential, or punitive damages. Our aggregate liability for a claim will not exceed the amount you paid us for the service giving rise to it. Nothing here excludes rights or liability that cannot legally be excluded.

10. Governing law and disputes

These Terms are governed by Wyoming law, without regard to conflict-of-law rules. Before filing a claim, contact us and allow 30 days for a good-faith resolution. Unless applicable consumer law requires otherwise, courts located in Sheridan County, Wyoming have exclusive jurisdiction.

11. Changes and contact
